following directions and information will be useful to you and
will assist us in making your first visit more enjoyable. We
respect your time and would like to make your visits to our office
as efficient as possible.
To ensure that your initial
visit goes quickly and smoothly, please make sure that your
referring physician has sent the necessary medical records. Our
office fax number us 214-739-5973. All medical records should be
sent to this number.
Medication refills and scheduling of appointments are handled
during normal office hours.
IMPORTANT
INSURANCE INFORMATION -- Please Read Carefully
Medicare Patients:
As required by law, we will file Medicare claims but we require
payment at the time of your office visit. Assignment is accepted for surgical,
chemotherapy and hospital service charges only.
PPO, HMO, and All
Other Insurance:

We do not file
insurance or accept assignment for office visits except for
those patients covered by a PPO or HMO plan with which we have a
contract or unless other arrangements have been made. All
payments, including co-pays, are due at the time services are
rendered.

It is imperative
that we have a copy of your current insurance card in your file.
Therefore, if you do not have your insurance card we will be
unable to file any charges with your insurance carrier and
payment for the office call and any procedure will be required
at the time of the visit.

Insurance payment is
expected within 60 days time or we will expect the patient to
make full and immediate payment.

We do not accept any
insurance carrier's determination of reasonable and customary,
allowable, or scheduled benefit as payment in full. We will be
happy to provide additional information to your insurance
company if requested. However, the remaining balance is the
responsibility of the patient.

We will accept
Mastercard and Visa as payment on your account. If you need to
make monthly payment arrangements, please call our office and we
will be happy to work with you.
HMO and Managed Care
Patients:
It is your
responsibility to be aware of the need for an authorization or
referral number from your primary care physician. This is especially
important if you are a new patient.
